Hey, that's from Satoshi Kon's "Paprika", which is fucking awesome. And the scene makes perfect sense in context, and is not meant to be pornographic in any way. (Notice that there's another woman's face under there. That is important.)

Also, I disagree on it not being pornographic. Just look at that cameltoe..The "cameltoe" does not exist until he puts his hand in. If you look at the rest of the animation, there is sort of a wake (like from a boat) where his hand is cleaving her body. That's what you're seeing.
It's been a while since I've seen the movie. I don't quite remember the scene actually... But it's not the type of movie where the intention is to be pornographic by any means. If you haven't seen it you should, because it's pretty great.

The film itself seems to be very metaphor heavy; the "fake skin" seems to be related to the subs that you can briefly seeright after the cuts.

Oh yeah, Paprika is great (although it makes me sound like a total anime nerd which I am not, the English voice acting is a little meh. The main character has too much of a "I AM VOICE ACTING IN ANIME OKAY?" quality to it). It's not pornographic at all, I saw it at our local Indie Theater. It's also got an entire scene involving Sphinxes, so I mean I think I'm bound by law to like it.

Anyway, about his foot, I think that in addition to being very fat he also has severe edema. Compression stockings might help with that, as will keeping his foot/feet above the level of his heart and possibly limiting his salt intake.
